OPH Attestation Form - Student Return from Illness

Ottawa Public Health (OPH) has released an Attestation for Return to School for Students Following Illness form.  To access the form please visit:   Attestation Form .  A copy of the form can also be found on the right-hand side of the landing page of our website shh.ocsb.ca .

Parents are asked to complete this form and provide it to the school when a child is returning to school due to illness.  Completed forms can be emailed to SacredHeart.High@ocsb.ca .  Alternatively, completed forms can be dropped off by students in the main office prior to returning to class.  

It is important to note that testing requirements for students who develop COVID-19 symptoms have not changed.  For your reference we are also including related information shared in our most recent school update:

OPH has posted a screening tool (click here) to help parents and guardians make decisions about whether their child can attend school/child care. This screening tool should be completed daily before attending school/child care for each individual child separately. 


When Your Child is Sick at Home


If your child is at home, experiencing any covid-related symptoms, they cannot attend school.  Please 

  • Inform the school at 613-831-6643 (main line) or 613-831-0543 (attendance line).  Please be sure to let the school know that the absence is due to COVID symptoms and whether testing will be done, or has been done.


If your child is home sick, they can access their school work via Hapara and/or their teacher's website. 


When Your Child Becomes Sick at School


If your child presents with symptoms consistent with COVID-19 while at school, the following steps will be taken, under OPH guidelines:

  • Student goes to isolation room

  • School calls parent/guardian to pick up student (and any siblings in the school)

  • Student and household contacts at the school get picked up to self-isolate at home

  • The school will provide families with a letter from Public Health, indicating guidelines and next steps.


Testing and Return to School


OPH advises that you take your child for COVID-19 testing if they experience symptoms.


If your child does get tested:

  • Your child must remain in isolation at home while waiting for the results of the COVID-19 test and cannot attend school in person. Siblings and other close contacts, such as people living in the same household, are also required to isolate at home while waiting for your child’s test results.

 

If your child tests negative:

  • Your child cannot return to school until at least 24 hours after symptoms have resolved without the use of fever-reducing medication, or 48 hours after vomiting or diarrhea has resolved

  • Household members can immediately stop self-isolation and go to school and work if they are symptom-free. 


If your child tests positive for COVID-19, OPH will receive the results directly from the  lab and will contact you to provide further direction and guidance. 


If your child does NOT get tested, your child and close contacts MUST stay home and self-isolate for:  

  • 14 days AND 

  • Not have a fever AND  

  • Symptoms have been improving for at least 72 hours, whichever is longer.


We appreciate these requirements may create challenges.  It is important to note, however, that these steps have been put in place by Ottawa Public Health to protect students, staff and families.
